Thyroid Eye Disease and Orbital Conditions

Thyroid eye disease is an autoimmune condition, most often linked to an overactive thyroid (Graves' disease). It affects the muscles and fat around the eye and can cause bulging eyes, eyelid retraction, redness, grittiness, double vision and, rarely, pressure on the optic nerve.

The orbit is the bony socket around the eye. Orbital conditions, such as inflammation or masses, need careful specialist assessment. Dr. Manar Al-Wuhaib, Consultant in Oculoplastic Surgery, provides this care, working alongside your endocrinologist and other specialists.

Who it's for

  • People with thyroid disease who notice eye changes, such as staring or bulging eyes, puffy lids or double vision.
  • Eye bulging or swelling around the eye from another cause.
  • Patients with stable thyroid eye disease who wish to discuss eyelid or orbital surgery.
  • Urgent: reduced vision or colour vision in thyroid eye disease needs to be seen promptly.

How it works

  1. Assessment: vision, colour vision, eye pressure, eye movements, measurement of eye position and lid position, and imaging when needed.
  2. Active phase: lubricating drops, protecting the eye surface and, when needed, medical treatment, coordinated with your thyroid doctor. Stopping smoking is strongly advised, because smoking is linked to more severe disease.
  3. Stable phase: in selected cases, surgery to correct eyelid position or reduce bulging. Your doctor will explain whether surgery is suitable and what it involves.

Recovery

  • Thyroid eye disease usually has an active phase, often lasting months, followed by a stable phase. Treatment is tailored to each phase.
  • Recovery after eyelid or orbital surgery depends on the procedure and is explained in detail at your consultation.
  • Regular follow-up is important, especially during the active phase.

Frequently asked questions

Will my eyes return to normal once my thyroid is treated?

Treating the thyroid is essential, but eye changes can continue on their own course. Some improve, while others need specific eye treatment.

Does smoking affect thyroid eye disease?

Yes. Smoking is linked to more severe disease and a weaker response to treatment. Stopping smoking is one of the most helpful steps.

When is it urgent?

Seek urgent care if your vision becomes blurred or colours look faded, or if you cannot close your eyes fully and the eye becomes painful.
